Chestnut Hill Rehabilitation & Healthcare Center is seeking a compassionate, organized, and resident-focused Social Services Assistant to join our interdisciplinary team. In this role, you'll work alongside our Social Services Director to support residents and families throughout their stay by assisting with admissions, care planning, discharge coordination, resident advocacy, and ensuring a positive resident experience.

Position Overview

The Social Services Assistant provides administrative and resident support to the Social Services Department while assisting with psychosocial services, admissions, discharge planning, care coordination, and resident advocacy. Working under the direction of the Social Services Director, this position helps ensure residents receive quality social services in accordance with federal, state, and facility regulations.

Key Responsibilities

  • Assist residents and families with adjustment to the facility and provide ongoing emotional support.

  • Welcome new admissions and help orient residents and families to the facility.

  • Assist with psychosocial assessments and required documentation as directed.

  • Participate in care plan meetings and interdisciplinary team meetings.

  • Help coordinate resident room changes and communicate changes with appropriate departments.

  • Assist with discharge planning and coordinate post-discharge follow-up calls within 48 hours.

  • Address resident and family questions, concerns, and requests, referring complex issues to the Social Services Director.

  • Attend Resident Council meetings and assist with follow-up on resident concerns.

  • Assist with admission paperwork, discharge documentation, and resident signatures.

  • Help maintain accurate resident records and required social services documentation.

  • Assist with PASRR, 408 forms, MA-51s, Notice of Non-Coverage letters, and other regulatory documentation as assigned.

  • Complete assigned MDS sections under the direction of the Social Services Director, when applicable.

  • Coordinate communication between residents, families, and the interdisciplinary care team.

  • Assist with facility tours and admissions support as needed.

  • Participate in marketing and community outreach activities when requested.

  • Maintain confidentiality of resident information and comply with HIPAA regulations.

  • Follow all facility policies, state regulations, and federal guidelines.

  • Perform other duties as assigned.

Qualifications

  • Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.

  • Previous experience in healthcare, long-term care, customer service, or social services preferred.

  • Experience working with the geriatric population is a plus.

  • Strong communication, organizational, and interpersonal skills.

  • Ability to demonstrate empathy, professionalism, and excellent customer service.

  • Basic computer proficiency and documentation skills.

  • Valid Pennsylvania Driver's License preferred.

  • Ability to lift up to 30 pounds (with or without reasonable accommodation).

Schedule & Status

  • Full-Time

  • Monday – Friday | 8:00 AM – 4:30 PM

  • Occasional weekends as needed

  • On-site position
