$38.00 hour to $42.00 hour Plus $7500 Sign On 

Sign-On Bonus: Up to $7500.00

DESCRIPTION

The RN Resident Care Coordinator oversees the Long-Term Living Units within the Facility. Demonstrate work habits that support the company’s mission to create and maintain a distinctive, service enriched environment and lifestyle for our Community’s residents and employees. Manages and supports the Community’s culture of providing superior hospitality service and amenities to the residents. Responsible for the daily functioning of the health care needs of the residents in the facility.


ESSENTIAL DUTIES & RESPONSIBILITIES:
·       Assure well-run departments. Serves as an ambassador of goodwill to all who enter the community.

·       Is aware of, and adheres to, Resident’s Bill of Rights and Confidentiality of Resident Information

·       Is aware of Resident Abuse Reporting Law

·       Maintains HIPAA privacy, security and confidentiality standards. Follows all established policies and procedures.

·       Follows universal Precautions and Infection Control techniques. Maintains Corporate Compliance standards.

·       Reports any noted changes in residents’ condition/behaviors or normal routine to Administrator/RN.

·       Arranges for clinical assistance for residents as needed per RN approval. (Restroom escorts, medication assistance, etc.).

·       Routinely communicates with Administrator and RN of any resident personal needs.

·       Reviews and documents on the service plan as needed.

·       Facilitate periodic family/resident service plan conference meetings to provide updates and changes of care. Works with nursing staff to update resident assessments and service plans. Interacts with families to assure their needs are being met. Attend service plan review as needed.

·       Oversees staff (Caregivers) that work in this area to ensure resident care, monitoring and appropriate interaction.


Qualifications:

  • Active Registered Nurse (RN) license in the state of New York.

  • Minimum of one year of experience in nursing administration or supervisory roles, preferably in long-term care, rehabilitation, or geriatric nursing.

  • Strong knowledge of healthcare regulations, including those specific to skilled nursing and rehabilitation settings.

  • Exceptional leadership, organizational, and communication skills.

  • Ability to foster a compassionate and efficient work environment.
