Director of Social Work (DSW) - Now Hiring

A respected long-term care organization is seeking an experienced Social Services Director to lead the Social Services Department at one of our skilled nursing facilities. This leadership role is responsible for overseeing department operations, ensuring regulatory compliance, and providing high-quality psychosocial support to residents and their families.

Key Responsibilities

• Provide leadership, direction, and oversight for the Social Services Department.

• Conduct resident assessments and develop individualized care plans.

• Assist residents and families with adjustment, support, and transition throughout their stay.

• Supervise, mentor, and support Social Services staff.

• Coordinate discharge planning beginning at the time of admission.

• Collaborate with the interdisciplinary care team to ensure quality resident outcomes.

• Participate in community outreach and develop relationships with external agencies as needed.

• Ensure compliance with all applicable federal, state, and facility regulations.

• Exercise sound judgment, initiative, and problem-solving skills in a fast-paced healthcare environment.

Qualifications

• Minimum of 2 years of management experience in a long-term care or skilled nursing setting.

• Current LSW required; MSW preferred.

• Strong knowledge of state and federal regulations governing long-term care.

• Excellent leadership, communication, and interpersonal skills.

• Ability to manage multiple priorities while maintaining a resident-centered approach.
