Certified Occupational Therapist

About the Position

We are seeking a licensed Occupational Therapist (OT) with a passion for improving patients’ quality of life in the comfort of their placement. The OT will help individuals enhance their ability to perform everyday activities and participate fully in home and community roles. Services are designed to promote health and wellness for individuals who have, or are at risk of developing, illness, injury, disability, or activity limitations.

Key Responsibilities

• Provide occupational therapy services to patients in home health settings.
• Address performance of daily activities and participation in meaningful roles.
• Develop and implement individualized treatment plans.
• Collaborate with patients, families, and interdisciplinary teams to promote independence and well‐being.
• Document care accurately and maintain compliance with professional standards.

Qualifications

• Active Occupational Therapist license in the state of Colorado.
• At least one year of experience in home health preferred.
• Strong written and verbal communication skills.
• Effective interpersonal skills and ability to build rapport with patients and families.
