Shift: Mon-Thurs 7AM-3:30PM (30)
Physical Therapist (PT) Duties & Responsibilities:
- Evaluate patients to assess mobility, strength, balance, and functional limitations.
- Develop and implement individualized treatment plans to improve mobility and reduce pain.
- Administer therapeutic exercises, manual therapy, and other rehabilitation techniques.
- Monitor patient progress and modify treatment plans based on clinical outcomes.
- Educate patients and caregivers on exercises, injury prevention, and home exercise programs.
- Assist patients in restoring function following injury, surgery, or illness.
- Maintain accurate and timely documentation of evaluations, treatments, and patient progress.
- Collaborate with physicians and interdisciplinary healthcare teams to coordinate care.
- Ensure compliance with facility policies, safety standards, and regulatory requirements.
- Promote patient independence and improve overall quality of life through rehabilitation services.
Minimum Qualifications
- Current, valid, and unrestricted CA license/certification in good standing, as required for the position.
- Minimum of 1 year of recent experience as a Licensed Physical Therapist .
- Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
- Copy of all current required certifications and compliance documents (e.g., . BLS, N95 fit test, immunization records, TB test, or other client-specific requirements ).
