Job Title: Hospice Social Worker (MSW)
Location: Zanesville, Ohio
Department: Hospice Care
Employment Type: Full-Time (40 hours/week)
Position Summary:
We are seeking a qualified and compassionate Hospice Social Worker (MSW) to join our interdisciplinary care team. The MSW will provide emotional, social, and resource support to patients and families facing end-of-life care.
Key Responsibilities:
Provide psychosocial assessments and ongoing support to patients and their families.
Collaborate with an interdisciplinary team including nurses, chaplains, and patient care technicians to ensure holistic care.
Assist families with care planning, advance directives, grief counseling, and resource coordination.
Maintain accurate documentation and comply with all regulatory standards set by the Ohio Department of Health (ODH).
Facilitate communication between patients, families, and healthcare providers.
Qualifications:
Master’s Degree in Social Work (MSW) – Required
Current Ohio licensure as a Social Worker (LSW or LPC) – Required
Minimum of one (1) full year of experience as a licensed Social Worker – Required (per ODH regulations)
Candidates with a Master's in Counseling are not eligible for this position due to state requirements.
