\n RN – Home Health | Roles & Responsibilities\n

Provide skilled nursing care to patients in their homes based on physician orders and individualized care plans.

\n

Conduct comprehensive patient assessments and develop, implement, and evaluate personalized plans of care.

\n

Complete OASIS assessments for admissions, recertifications, resumptions of care, and discharges.

\n

Serve as the Case Manager for assigned patients, coordinating care with physicians and the interdisciplinary team.

\n

Monitor patient progress, administer medications and treatments, and provide disease management.

\n

Educate patients and caregivers on medications, treatments, safety, and self-care techniques.

\n

Coordinate referrals and community resources to ensure continuity of care.

\n

Maintain accurate and timely documentation using Homecare Home Base (HCHB) and comply with agency and regulatory standards.

\n

Participate in weekend rotation (approximately every 4th weekend) and provide high-quality, patient-centered home health services.

Required Documents / Certifications\n

• Active RN License

• BLS

• COVID-19 Vaccination (Required – No Exemptions)

• Home Health experience (Required)
\n
• OASIS experience (Required)
\n
• Home care traveler experience with OASIS (Preferred)
\n
• Homecare Home Base (HCHB) experience (Preferred)
