Type: Full-time | Salary: Starting at $80,000.00+ per year

Responsibilities:

• Admit and reassess home health patients
• Coordinate and supervise Home Health Aides
• Conduct home care visits and reassess regularly
• Implement care plans and document progress
• Educate patients and caregivers

Requirements:

• NYS RN License
• Positive collaboration skills
• Strong communication and time-management
• Ability to travel daily
• COVID-19 vaccination required

Language Skills: Bilingual in English & Spanish (preffered)

Certifications:

• BLS Certification
• RN License

Location: On the road
