Working under the supervision of a Registered Nurse, the LVN plays a critical role in supporting patients and families through end-of-life care while collaborating closely with the interdisciplinary team.Key ResponsibilitiesDeliver skilled nursing care in accordance with the plan of care (POC) under RN supervisionSupport the RN in implementing and coordinating patient care needsAssist patients and families with education on self-care and disease managementDocument all clinical care, interventions, and patient responses accurately and timelyCommunicate changes in patient condition to the RN and care teamAssist with procedures and specialized care related to end-of-life supportPrepare and maintain equipment and supplies using proper aseptic techniquesEnsure care is delivered in compliance with organizational policies and regulatory standardsPromote the mission, values, and quality standards of Hospice of South TexasParticipate in interdisciplinary collaboration to ensure continuity of carePerform other duties as assignedQualificationsEducation & LicensureGraduate of an accredited vocational nursing programCurrent Licensed Vocational Nurse (LVN) license in the State of TexasValid Texas Driver's LicenseExperienceMinimum of 2 years of clinical nursing experienceHospice or home health experience preferredSkills & AbilitiesStrong clinical nursing skills aligned with accepted standards of practiceExcellent communication and interpersonal skillsAbility to respond effectively in emotionally sensitive situationsStrong documentation and organizational skillsOther RequirementsReliable transportation and valid auto liability insuranceProof of Hepatitis B vaccination or declinationWork Environment & Physical RequirementsProvide care in patients' homes with varying conditionsFrequent standing, walking, bending, and lifting (up to 50 lbs.)Exposure to bloodborne pathogens and infectious diseasesAbility to work flexible schedules, including on-call or PRN emergency situationsLocal travel requiredWhy Join HOST?Mission-driven organization focused on meaningful patient careSupportive and collaborative team environmentOpportunity to make a lasting impact in patients' livesCommitment to delivering 5-star quality careApply TodayIf you are a compassionate LVN looking to make a difference in hospice care, we encourage you to apply and join our dedicated team.
