Responsibilities:

• Wound vac management and monitoring
• Skilled nursing care in the home setting
• Medication administration and treatments
• Monitoring and documenting patient condition
• Communicating with physicians, families, and care teams
• Ensuring patient comfort, safety, and quality of care

Requirements:

• Current Texas LVN license in good standing
• Wound vac experience required
• Strong clinical assessment and communication skills
• Home health or private duty experience preferred
• Current CPR certification preferred
• Reliable transportation
