About the Role:

As a Physical Therapist (PT) at Rock River Nursing & Rehab, you will be responsible for providing high-quality, patient-centered care to individuals with a variety of conditions, including chronic illnesses, injuries, and post-operative rehabilitation. You will design and implement individualized treatment plans, conduct evaluations and assessments, and collaborate with our interdisciplinary team to ensure comprehensive care.

Responsibilities:

• Conduct patient evaluations and assessments to identify physical therapy goals and objectives

• Develop and implement individualized treatment plans to achieve patient goals

• Provide direct patient care, including exercises, stretches, and other interventions

• Monitor patient progress and adjust treatment plans as needed

• Communicate with patients, families, and healthcare teams to ensure continuity of care

• Participate in interdisciplinary team meetings to discuss patient care and progress

• Maintain accurate and detailed records of patient care and treatment plans

Requirements:

• Bachelor's or Master's degree in Physical Therapy

• Current licensure as a Physical Therapist

• Minimum of 1 year of experience

• Knowledge of current best practices in physical therapy and patient care
