JNOW HIRING a Physical Therapy Assistant for per diem OR part-time opportunities in the home care setting. You will be paid for each completed patient visit (with documentation).

WORK INDEPENDENTLY  *  1-to-1 IN-HOME PATIENT CARE

RESPONSIBILITIES: 

• Perform home visits for patients in a home care setting
• Under the supervision of the Physical Therapist, performs treatment modalities as indicated according to the plan of care
• Implements plan of care to provide safe, efficient quality care to patients on a daily basis, reporting any changes to the Physical Therapist or Clinical Manager.

QUALIFICATIONS:

• Current license as a Physical Therapy Assistant in Connecticut
• Valid driver’s license
• Reliable transportation and willing to travel in New London and/or Windham and/or Middlesex Counties
• Strong communication and interpersonal skills
• Home health experience including Medicare preferred

Graduate of a physical therapy school approved by: Commission on Accreditation in Physical Therapy Education-accredited physical therapist assistant education program and certified in Connecticut.
