Graduate of a physical therapy school approved by: Commission on Accreditation in Physical Therapy Education-accredited physical therapist assistant education program and certified in Connecticut.

RESPONSIBILITIES:
- Perform home visits for patients in a home care setting
- Under the supervision of the Physical Therapist, performs treatment modalities as indicated according to the plan of care
- Implements plan of care to provide safe, efficient quality care to patients on a daily basis, reporting any changes to the Physical Therapist or Clinical Manager.
QUALIFICATIONS:
- Current license as a Physical Therapy Assistant in Connecticut
- Valid driver’s license
- Reliable transportation and willing to travel in New London and/or Windham and/or Middlesex Counties
- Strong communication and interpersonal skills
- Home health experience including Medicare preferred
