Responsibilities:

• Provide direct patient care, including administering medications, performing treatments, and documenting patient records

• Develop and implement personalized care plans in collaboration with other healthcare professionals

• Monitor patient status and report any changes or concerns to the healthcare team

• Collaborate with other departments, such as physical therapy and occupational therapy, to ensure comprehensive care

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Maintain accurate and detailed records of patient care and interactions

• Educate patients and families on disease management, medication usage, and other aspects of care
