Physical Therapist (PT)

We are seeking a motivated and compassionate Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist will evaluate, plan, and provide skilled physical therapy services to residents within our skilled nursing facility. This role focuses on helping residents achieve their highest functional level, improve mobility, reduce pain, and enhance quality of life.

Responsibilities:

• Evaluate residents’ functional abilities and develop individualized treatment plans.

• Provide direct physical therapy treatments to improve strength, mobility, balance, endurance, and coordination.

• Utilize a variety of therapeutic techniques, exercises, and equipment to promote functional independence.

• Collaborate with the interdisciplinary team to ensure continuity of care and effective treatment outcomes.

• Supervise and direct Physical Therapy Assistants (PTAs) and therapy aides in carrying out treatment plans.

• Monitor and document residents’ progress, adjusting treatment plans as needed.

• Educate residents, families, and caregivers on therapy techniques, equipment use, and strategies for continued progress.

• Ensure compliance with all federal, state, and facility regulations and professional standards.

• Participate in discharge planning by providing recommendations for ongoing therapy or equipment needs.

Qualifications:

• Graduate of an accredited Physical Therapy program.

• Current licensure as a Physical Therapist (PT) in the state of Wisconsin.

• Prior experience in a skilled nursing facility or similar healthcare setting preferred, but not required.

• Strong clinical, organizational, and communication skills.

• Ability to work collaboratively with the healthcare team while also managing individual caseloads.

• Compassionate, patient, and resident-centered approach to care.
