Home Care Registered Nurse (RN) – Job Description

We are seeking a compassionate and experienced Home Care Registered Nurse (RN) to join our team. The ideal candidate will provide high-quality, patient-centered care in a home setting and play a key role in improving patients’ health outcomes and quality of life.

Responsibilities:

• Provide skilled nursing care to patients in their homes

• Assess patient conditions and develop individualized care plans

• Monitor and document changes in patient health, symptoms, and behavior

• Administer medications and treatments as prescribed

• Collaborate with physicians and other healthcare professionals to coordinate care

• Educate patients and families on health maintenance and disease prevention

• Facilitate referrals to specialists and community resources as needed

• Maintain accurate and timely medical records

• Offer emotional support and guidance to patients and their families

Qualifications:

• Active Registered Nurse (RN) license in the state of Virginia

• Previous nursing experience required; home care experience preferred

• Strong clinical assessment and critical thinking skills

• Familiarity with electronic medical records and medical equipment

• Excellent communication and interpersonal skills

• Ability to build rapport with patients and families

• Ability to work independently and manage time effectively in a home care setting
