Registered Nurse - Home Care - Field Supervisor

We are seeking Full time RN FIELD SUPERVISORS in Nassau and Suffolk counties. As a field supervisor, you will further Bestcare’s goal for our patients to remain safely in their homes, functioning at  their optimum level of independence.

Job Responsibilities:

• Conduct initial home visits to assess patient needs, develop the Aide Plan of Care, and provide ongoing oversight and support
• Communicate with a patient’s physician, insurance, and other team members to ensure delivery of appropriate patient care
• Supervise patient care provided by our Home Health and Personal Care Aides
• Participate in orientation, teaching, and evaluations of care providers
• Responsible for the review and maintenance of patient records

Job Qualifications:

• Must be a valid Registered Nurse licensed in New York State, preferably with two (2) years of experience
• Must have a valid driver’s license and transportation
• Previous home care experience is a plus
• Knowledge of EMR HHAX a plus
• Bilingual English Spanish is a plus
