Nurse Practitioner (NP) – Nursing Home / Primary Care
Full-Time | Direct Hire | Day Shift
Salary: $135K–$150K + Benefits
Seeking a dedicated NP to provide high-quality care in a nursing home setting. New graduates are encouraged to apply. Responsibilities include patient assessments, care planning, chronic/acute condition management, documentation using EHR systems, and collaboration with interdisciplinary teams.
Requirements
- Master’s Degree in NP Program (Family or Adult/Geriatrics)
- Active New York State NP License
- National Board Certification (AANP, ANCC, or ONCC)
- 1+ year experience preferred
- EMR/EHR experience preferred
- Spanish-speaking is a plus
