Job Title: Licensed Practical Nurse – Primary Care Clinic
Job Description
This role provides hands-on clinical support in an ambulatory care setting, delivering high-quality patient care, coordinating patient flow, and supporting providers during clinic visits. The position focuses on patient intake, vital signs, clinical documentation, and lab coordination while contributing to a supportive, team-oriented environment that emphasizes health, wellness, and excellent patient experience.
Responsibilities
- Provide direct patient care in an ambulatory clinic setting and prepare patients for provider visits.
- Obtain and record vital signs accurately and document all relevant clinical information in the electronic medical record (EMR).
- Perform patient intake, including verifying medical history, medications, and chief complaints, and assist with patient flow throughout the clinic.
- Conduct clinical tasks as directed by providers, ensuring safe, efficient, and patient-centered care.
- Manage patient phone calls and voicemails, documenting cases clearly and routing information to the appropriate clinical staff.
- Place laboratory and diagnostic orders under provider direction and ensure accuracy in order entry.
- Prepare lab requisitions and support lab workflow for approximately 30 patients per day, including proper labeling and handling of specimens.
- Assist with patient education by reinforcing provider instructions, explaining follow-up plans, and promoting health and wellness.
- Maintain exam room cleanliness, organization, and readiness, including routine stocking and supply ordering.
- Support overall clinic operations by collaborating closely with nurses, providers, and other staff to ensure a smooth, efficient patient experience.
- Participate in care management and case management activities as appropriate to support patient continuity of care.
- Promote a positive, family-oriented team culture and contribute to a respectful, collaborative work environment.
Essential Skills
- Active Licensed Practical Nurse (LPN) license in the State of Virginia, as required by the role.
- Graduation from an accredited school of practical nursing.
- Minimum of 3+ years of recent medical/surgical nursing experience.
- At least 3+ years of clinical experience in a medical office or similar outpatient setting.
- Current BLS/CPR certification.
- Demonstrated experience in patient care and care management within a healthcare environment.
- Ability to work effectively in ambulatory and primary care–type settings.
- Strong multitasking ability to manage a high-volume clinic environment and approximately 30 patients per day in lab workflow.
- Proficiency with electronic medical records, specifically Athena EMR or similar systems.
- Excellent communication skills with patients, families, and interdisciplinary care teams.
- Strong organizational and time management skills to prioritize tasks and maintain efficient patient flow.
- Ability to thrive in a fast-paced, high-volume setting while maintaining attention to detail.
- Team-oriented mindset with a proactive, solutions-focused approach to daily responsibilities.
Additional Skills & Qualifications
- Primary care experience in an outpatient or ambulatory clinic is preferred.
- Experience in health care settings focused on nutrition, health, and wellness is beneficial.
- Background in health and wellness programs or initiatives is a plus.
- Comfort working within case management or care management frameworks to support patient outcomes.
- Ability to provide patient education on wellness, prevention, and follow-up care.
- Strong interpersonal skills, including empathy, patience, and the ability to build trust with patients.
- Comfort collaborating across clinical staff and disciplines in a team-based care model.

Work Environment
This position is based in a clinic environment serving locations such as Elizabeth City, Camden, and Edenton. The schedule is Monday through Friday, typically 8:00 AM to 4:30/5:00 PM, for a 40-hour workweek. The role is structured as contract-to-hire, offering an opportunity to transition into a longer-term position based on performance and organizational needs. The clinic uses Athena as its electronic medical record system, and staff rely on standard clinical equipment for vital signs, lab preparation, and patient care. The culture emphasizes a supportive, family-oriented atmosphere with strong collaboration across clinical staff.
