Job Summary:

NuChoice Health Partners, a leading home care agency serving the Northwest of Pennsylvania, is seeking compassionate and trustworthy Direct Care Workers to join our team. As a Direct Care Worker, you will provide personalized care and support to our clients in the comfort of their own homes, promoting their independence, dignity, and overall well-being.

Responsibilities:

• Provide companionship and social support to clients, engaging in activities such as meal prep, errands, and light housekeeping

• Assist with medication reminders, managing medications, and ensuring timely administration

• Help with personal hygiene and grooming, promoting independence and autonomy

• Develop and maintain positive relationships with clients, families, and caregivers

• Document client care activities and progress, maintaining accurate records and reporting to supervisors as needed

Requirements:

• 0 years of experience (or equivalent training) in healthcare, customer service, or a related field

• Ability to work part-time and full-time schedules, including evenings, weekends, and holidays

• Reliable transportation and a valid driver's license (if applicable)

• Ability to lift, bend, and move client supplies and equipment (up to 50 pounds)

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team
