About who needs care: Diagnised with Creutzfeldt Jakob disease, a rapid and aggressive neurodegenerative disease of the brain. Moderate confusion and memory loss which will progress some instability when walking, signs of dysphagia, 2 small dogs in the home. She is a busy person, likes to clean and work in her yard. About the care needs: Currently using Affinity for medical care with an aide, nurse, and social worker who come 3-5 times per week. Services needed include: transportation, meal preparation, errands / shopping, light housekeeping, companionship, feeding, and bathing / dressing.
