Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• CPR (Basic) is required to start.
• Hepatitis B documentation is required to start.
• TB documentation is required to start.
• Flu documentation is required to start.
• Information Security Compliance Form is required to start.
• Orientation - Review Annually is required to start.
• Signed Job Description is required to start.
• Completed Orientation Documents is required to start.

Assignment Details

• Shift schedule: 5x8.
• Hours: 12:00 AM - 12:00 AM.
