Position Summary
The MDS Coordinator is responsible for the accurate and timely completion of the Minimum Data Set (MDS) process in accordance with federal and state regulations. The successful candidate will collaborate with interdisciplinary team members to ensure comprehensive resident assessments, appropriate care planning, and optimal reimbursement outcomes.

Qualifications

• Active Registered Nurse (RN) license in the State of New Jersey required
• Minimum one (1) year of experience in MDS processing required
• Strong knowledge of Medicare, Medicaid, and MDS regulations
• Excellent organizational, communication, and leadership skills
• Skilled Nursing Facility (SNF) experience preferred

Responsibilities

• Coordinate and oversee the MDS assessment process
• Ensure timely and accurate completion and submission of all MDS assessments
• Work closely with the interdisciplinary team to develop and update resident care plans
• Monitor regulatory compliance and reimbursement opportunities
• Participate in quality assurance and performance improvement initiatives
• Serve as a member of the Skilled Nursing Leadership Team
