About the Position
We are seeking a skilled and compassionate Pediatric Home Care Nurse (RN or LPN) to provide one‐on‐one care for a medically complex adolescent. The ideal candidate is dependable, attentive, and experienced in caring for technology‐dependent or neuromuscular pediatric patients. This role supports the patient’s daily medical needs, safety, and overall well‐being within the home environment.
Primary Diagnoses
- Duchenne muscular dystrophy
- Neuromuscular scoliosis
- NG‐tube dependence
- Restrictive lung disease
- Neuromuscular cardiomyopathy
Responsibilities
- Provide skilled nursing care according to the patient’s individualized care plan
- Perform ongoing assessments of respiratory, cardiac, nutritional, and neuromuscular status
- Manage NG‐tube feedings, medication administration, and troubleshooting
- Monitor respiratory function, oxygen saturation, and secretion management
- Assist with airway clearance techniques (cough‐assist, CPT, suctioning as ordered)
- Support safe positioning, transfers, and range‐of‐motion exercises
- Monitor skin integrity and prevent pressure injuries
- Document all care accurately and promptly
- Track nutritional intake, hydration, and weight trends
- Report any changes in condition or safety concerns to the supervising nurse and medical team
- Collaborate with family members and multidisciplinary providers
- Maintain a safe, supportive, and respectful home environment
Qualifications
- Active RN or LPN license (state‐specific)
- Pediatric experience required
- Experience with medically fragile or technology‐dependent patients preferred
- Competency in NG‐tube care and respiratory monitoring
- Strong assessment and communication skills
- Ability to work independently in a home setting
- Understanding of mandatory reporting responsibilities
Preferred Experience
- Neuromuscular disorders
- Airway clearance devices
- Cardiac monitoring for neuromuscular cardiomyopathy
- Trauma‐informed care or experience supporting vulnerable youth
Schedule
- Nights and weekend
