Description\n

American Traveler is hiring a Home Health RN with required OASIS and home health experience for a Case Manager role requiring a VT or compact RN license.

Details\n

\n
• Home health and hospice care setting serving patients of all backgrounds
\n
• Serves as Case Manager for cases where this role is the lead discipline
\n
• Day shift schedule Monday through Friday
\n
• Weekend rotation required approximately every 4th week
\n
• Homecare Home Base experience preferred
\n

Requirements\n

\n
• Active VT or compact RN license required
\n
• BLS certification required
\n
• Minimum of 1 year of experience required
\n
• Home health experience required
\n
• OASIS experience required
\n
• Experience as a home care traveler with OASIS proficiency preferred
\n
• COVID-19 vaccination required with no exemptions
\n

• Provide individualized, goal-oriented nursing care using the nursing process in accordance with physician orders
\n
• Coordinate care as Case Manager when serving as the lead discipline
