Wapak Manor is under new management and building a strong, dedicated team. We are looking for a Physical Therapist (PT) who wants to be part of a facility that is focused on improving care, teamwork, and outcomes.

• Evaluate and treat residents to improve mobility and strength

• Develop and implement individualized care plans

• Work closely with nursing and interdisciplinary team

• Maintain accurate and timely documentation

• Educate residents and families on therapy plans

• Licensed Physical Therapist in the state of Ohio

• Long-term care experience preferred (not required)

• Strong communication and organizational skills
