Schedule- Monday-Friday, 8:00 AM - 5:00 PM
- Only 2 holiday on-call shifts per year

As an RN Case Manager, you will serve as the primary nurse for your assigned patients and families, providing compassionate care and guidance throughout their hospice journey.
Responsibilities include:
- Managing and coordinating patient care plans
- Completing comprehensive patient assessments and determining hospice eligibility
- Leading hospice admissions and care planning
- Collaborating with physicians and interdisciplinary care teams
- Assessing and addressing physical, emotional, social, and spiritual needs
- Conducting routine patient visits and updating care plans as needed
- Providing support and education to patients and families
- Maintaining accurate clinical documentation
- Supervising Home Health Aides in accordance with regulatory requirements
- Providing compassionate end-of-life support and clinical pronouncements when necessary
- Graduate of an accredited Registered Nursing program
- Active RN license in the state of Ohio
- Current CPR and First Aid certification
- Strong clinical assessment and decision-making skills
- Excellent communication and relationship-building abilities
- Passion for hospice and patient-centered care
- Valid driver's license and reliable insured transportation
