The Community Health Worker provides peer services, including conducting outreach and engagement efforts, assisting patients in self-advocacy and self-management, accompanying patients on clinical and supportive service appointments, and assisting the 50PLUS and AI MRT teams during specified interventions and health education sessions. They work with patients throughout the region, including Niagara, Erie, Chautauqua, Cattaraugus, Allegany, Wyoming, Genesee, and Orleans counties.
As part of the essential functions of this role, the Community Health Worker:
- Assists with development and implementation of the annual outreach plan, which includes face-to-face meetings, tabling events, and other goals and objectives for reaching the target population
- Works with participants in designated programs to link them to care coordination and HIV medical care; assists participants with appointment reminders and accompanies participants to appointments if needed
- Conducts outreach according to strategies identified in the annual plan, grant workplan and the enhanced outreach model
- Reaches out to enrolled patients who are not virally suppressed to discuss barriers and options to reengage in care
- Assists with referrals and linkages and assists with initial engagement in needed Evergreen supportive services
- Reaches out to patients to re-engage them in care, including sending letters, conducting home visits, or meeting patients at a medical appointment
- Tracks progress in work plans, and provides additional administrative and patient support based on program needs
Qualified candidate will have a high school diploma/GED (preferred); personal or professional experience with HIV/Aging communities; strong communication skills; ability to do outreach at various times in a variety of locations; sensitivity to HIV/AIDS, addiction, and LGBTQIA+ issues and awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
Job type: Part-time, 20 hours per week; also open to full-time, based on candidate's interest/availability
Education/Experience: High school diploma/GED (preferred); personal or professional experience with HIV/Aging communities
