Registered Nurse (RN) – Home Health
\n Pride Health is seeking a Registered Nurse (RN) – Home Health to join a leading healthcare organization in Fairview Heights, IL.
Responsibilities
\n •Complete OASIS assessments, follow-up visits, and comprehensive patient evaluations in the home setting.
\n •Provide skilled nursing care, including wound care, IV therapy, and follow-up visits.
\n •Document patient care accurately and efficiently using Homecare Homebase (HCHB).
Qualifications
\n • Active Registered Nurse (RN) license.
\n • Recent Home Health nursing experience required.
\n • Experience with Homecare Homebase (HCHB) required.
\n • Previous experience with an LHC Group agency preferred.

\n • Employment Type: 14-week contract
\n • Location: Fairview Heights, IL
\n • Start Date: ASAP
\n • Schedule: Monday–Friday | 8:00 AM – 4:30 PM
\n • Weekly Hours: 32-hour guarantee
\n • On-Call/Weekend Requirement: On-call rotation during weekdays and weekends
\n • Coverage Area: Red Bud, IL service area
\n • Pay Range: $53.00–$57.00 per hour (based on experience)
\n • Mileage Reimbursement: Yes, mileage is reimbursed between patient visits.
