Requirements

• Minimum 2 years of Long Term Care nursing experience

• BLS certification required

• Every other weekend (EOW) required

Responsibilities\n

• Coordinate and manage care for long-term care residents.
\n
• Perform comprehensive nursing assessments and administer medications and treatments.
\n
• Monitor changes in residents' conditions and promptly communicate with providers.
\n
• Develop and implement individualized care plans.
\n
• Provide education and emotional support to residents and their families.
\n
• Maintain accurate documentation and collaborate with the interdisciplinary healthcare team.
\n
• Ensure compliance with facility policies and quality standards.
