EDUCATION, QUALIFICATIONS AND CREDENTIALS
Must possess a 4-year college degree, ecclesiastical endorsement and at least two units of Clinical/Spiritual Education or equivalent. Demonstrates the ability to work effectively with all levels of personnel. Experience includes at least 5 years of pastoral/spiritual care ministry. Demonstrates rudimentary knowledge of, or experience in, the operation of long-term care in the state of Kentucky. Demonstrates a knowledge and respect of religious beliefs/practices of the faith groups which might potentially become residents of/employees of the facilities as outlined by Title VI of the civil rights code and maintains a good rapport with clergy from various faith traditions. Demonstrates a high level of confidentiality.
JOB ROLE
- Recognize your role as part of the QAPI efforts of your organization.
- Attend trainings to build understanding and capacity to undertake QAPI work.
- Carry out QAPI roles and responsibilities as assigned.
- Follow established policies and procedures in support of QAPI efforts.
- Look for, and share with leadership, ideas for improvement in the organization.
- Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success.
- Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.
ESSENTIAL JOB FUNCTIONS
- Consistently model and demonstrate excellence in customer service, with a constant focus on person-centered care/services delivery.
- Ensures adherence to the mission and philosophy of the organization.
- Ensures that the rights of residents entrusted to the organization are respected.
- Provides pastoral care to residents, their families, visitors and staff to facilitate the healing process.
- Completes a “Spiritual Assessment” of each new resident within 7 days of admission.
- Visits residents a minimum of every 90 days and create a chaplain note in PCC.
- Responds to special needs or requests of spiritual care for any new or current resident of the facility.
- Plans, coordinates and facilitates chapel services, staff devotions and Bible studies on a regular schedule, either by leading these programs or by securing leadership of the program.
- Participates in the development and implementation of facility policies and procedures when requested.
- Maintains appropriate communication with site management and all staff members.
- Attends staff/management meetings, respecting that the spiritual immediacy of the position may require absence from some.
- Assists or implements the contact with the resident’s local church, as soon as possible upon admission, and change in health status or death.
- Ensures that the spiritual needs of the facility are met on a 24-hour, 7 day a week basis through own service or that of assistant chaplains.
- Actively participates in morning staff huddles, QAPI and orientation of new employees as a member of the interdisciplinary team.
