Speech-Language Pathologist (SLP) — Full Time, Multi-Site Coverage
Location: Buffalo, NY and surrounding areas (three skilled nursing facilities) Employment Type: Full Time, Monday–Friday Pay Rate: $48.00 per hour Travel: Mileage reimbursement provided for travel between assigned sites
Position Summary
We are seeking a full-time Speech-Language Pathologist to serve as a floating coverage clinician across three of our skilled nursing facilities in the Buffalo area. This role is ideal for an SLP who enjoys variety, works well independently, and is comfortable stepping into different buildings and interdisciplinary teams. Hours are split across the three sites based on caseload and coverage needs, with schedules set in advance.
Responsibilities
Perform comprehensive speech, language, cognitive-communication, and swallowing evaluations for short-term rehab and long-term care residents
Develop and implement individualized treatment plans with measurable, functional goals
Provide dysphagia management, including diet texture and liquid consistency recommendations, compensatory strategies, and staff/family education
Coordinate with physicians, nursing, dietary, and rehab colleagues on plan-of-care decisions
Contribute to MDS assessments and support accurate PDPM classification
Complete timely, compliant documentation (evaluations, daily notes, progress reports, discharge summaries) in the facility EMR
Screen residents for changes in status and respond to referrals across all three assigned buildings
Supervise SLP Clinical Fellows or graduate students as assigned
Participate in care plan meetings, rehab meetings, and QAPI activities as needed
Maintain compliance with Medicare Part A and Part B requirements, state regulations, and facility policies
Qualifications
Required
Degree in Speech-Language Pathology from an accredited program
Current New York State SLP license (or license-eligible)
Valid driver's license, reliable transportation, and willingness to travel between the three assigned Buffalo-area sites
Ability to work independently and adapt to different facility teams and workflows
Preferred
Prior experience in skilled nursing, subacute rehab, or geriatric care
Experience with dysphagia management and instrumental assessment interpretation (MBSS/FEES)
Familiarity with PointClickCare or comparable long-term care EMR
Working knowledge of MDS, PDPM, and Medicare therapy documentation standards
Compensation and Benefits
$48.00 per hour
Mileage reimbursement for travel between assigned facilities during the workday
