Home Health Aide Patient Escort

Brooklyn Center is actively seeking Home Health Aides to serve as Patient Escorts for our Skilled Nursing Facility in Brooklyn, NY.

 
DUTIES:

• Escort residents to and from appointments

• Observing and monitoring Residents.

• Reporting any health issues to the supervising nurse.

• Maintain confidentiality of all Resident & Facility data.

REQUIREMENTS:

• Valid New York State HHA Certification.

• Occasional heavy lifting may be required.

• Ability to follow the Plan of Care precisely! 

• Must be able to work as a team member

• Valid photo id
