Minimum Qualifications:

• Current New York State Home Health Aide (HHA) certification

Job Duties:

• Escort residents to and from clinic appointments and dialysis sessions
• Monitor and prevent falls and altercations, intervening immediately
• Assist residents in preparing for appointments and ensure they receive snacks during out-on-pass periods
• Report incidents and accidents promptly to the clinical coordinator or ADNS
• Act as a representative of the facility, adhering to all policies and procedures on and off the property
