Responsibilities
- Conduct comprehensive OT evaluations for acute care patients and develop individualized treatment plans to restore and improve physical, social, and cognitive functions
- Provide evidence-based OT interventions using established protocols and client-centered approaches
- Collaborate as a co-leader within an interdisciplinary team; support patient groups as needed
- Establish and communicate individualized discharge plans and ensure continuity of care with appropriate parties
- Complete comprehensive and timely clinical documentation, including evaluations, progress notes, and outcomes
- Supervise assigned OT assistants and ensure adherence to professional standards and safety

Requirements
- Education: Master's degree in Occupational Therapy or Doctor of Occupational Therapy (OTD) required; OTD required if OT license obtained after January 2007, or foreign equivalent
- Licenses/Certifications: Occupational Therapy licensure in all applicable states of practice required; current BLS/CPR certification required
- Experience: At least two years of recent experience practicing OT
- Other: Valid Driver’s License and must be at least 21 years of age; must know HCHB and EPIC
- Additional notes: Home health experience is preferable but not required
