Job Description
Healing Care Hospice Inc is seeking a Registered Dietician to support patient nutrition care within a compassionate hospice setting.
Position SummaryThe Registered Dietician will assess patients’ nutritional needs and develop individualized nutrition plans that support comfort, health outcomes, and quality of life. This role partners with clinical teams to provide guidance on diet modifications, nutrition goals, and ongoing monitoring.
You will bring evidence-informed nutrition expertise to a hybrid work environment while collaborating with interdisciplinary caregivers to ensure nutrition care plans are followed consistently and updated as needs change.
Key ResponsibilitiesConduct nutrition screenings and assessments, including review of medical status, dietary intake, and relevant history.
Develop and implement individualized nutrition care plans aligned with patient goals and care requirements.
Provide recommendations to patients, families, and care teams regarding diet modifications, supplementation, and symptom management.
Monitor nutritional progress and adjust care plans based on changes in condition, intake, and tolerance.
Maintain accurate, timely clinical documentation and communicate nutrition updates to the interdisciplinary team.
Registered Dietitian (RD/RDN) credential.
Experience providing nutrition assessment and care planning in a clinical setting.
Strong communication skills with the ability to explain nutrition guidance clearly to patients and families.
Ability to work collaboratively with interdisciplinary teams and follow care plan directives.
Commitment to accurate documentation and patient-centered care.
Experience in hospice, palliative care, geriatrics, or long-term care.
Familiarity with nutrition support considerations and symptom-focused dietary planning.
Demonstrated experience using clinical judgment to tailor interventions to patient comfort and goals.
Experience providing guidance and education to caregivers and interdisciplinary staff.
Hourly salary: $40.00 - $53.00
