• Assist with mobility and ambulation, including transferring residents to and from wheelchairs and beds

• Monitor and report changes in residents' conditions, including vital signs and behavior

• Support residents with daily living activities, such as feeding and medication management

• Collaborate with other healthcare professionals to develop and implement individualized care plans

• Maintain accurate and up-to-date records of resident care and activities
