• Per diem role

• This is a multi-facility role

• Remote

• 2 weekends a month

• Some holiday coverage

• May be asked to cover for full time CE- vacations
  

Responsibilities:

• Conduct thorough clinical evaluations for patients admitted to our skilled nursing and rehabilitation programs

• Develop and implement individualized care plans in collaboration with healthcare teams

• Monitor patient progress and make adjustments to care plans as needed

• Provide medical direction and guidance to nursing staff

• Participate in quality improvement initiatives and contribute to the development of clinical protocols

• Collaborate with interdisciplinary teams to ensure seamless patient care

• Maintain accurate and up-to-date records and documentation
