We are currently looking for an experienced Start of Care/Eval Physical Therapist (PT) to join our team in the

Southeastern Houston area and surrounding areas.

Job Summary:
Looking for a FullTime Physical Therapist professional who can provide Oasis, Evaluations and Re evaluations to patient in the Southeastern Houston area and then hand off care to PTAs.


Job Qualifications:
Education: Bachelor of Science in Physical Therapy
Licensure: Current Texas State License as a Physical Therapist
Experience:
1 year of minimum experience as a Physical Therapist in Home Healthcare, Acute care, or Rehabilitation setting
Skills:
Therapy skills as defined by generally accepted standards of practice.
Good communication and interpersonal skills.
Proof of current CPR and Hepatitis profile.

Knowledge of durable medical equipment
Spanish is a Plus


Essential Functions:
Perform initial and ongoing clinical assessments to identify patient needs, determine the level of function at appropriate time points, and assess the appropriateness of the patient for Home health
Develop and revise a plan of care in consultation with the physician and other care team membe

Evaluate outcomes of care with appropriate discharge planning.
Apply concepts of infection control and standard precautions incoordinating/performing patient care activities to prevent contamination andtransmission of disease
Accept clinical assessments that are consistent with education and competence to carefor patients.
Supervise PTA as needed
Complete appropriate documentation in a timely manner
Meet the mandatory continuing education requirements of the agency/license board
Use effective interpersonal relations and communication skills to advise and consultwith the family and other agency personnel as appropriate
Demonstrate commitment, professional growth, and competency
Promote Agency philosophy and administrative policies to ensure quality of care
