Discharge Planner / Social Worker

Fairview Skilled Rehabilitation & Nursing Center is seeking a compassionate and organized Discharge Planner / Social Worker to coordinate safe and timely discharges for our residents. This position works closely with residents, families, physicians, nursing, therapy, and community providers to ensure a smooth transition from our facility to the next level of care.

Responsibilities

- Coordinate discharge planning from admission through discharge.

- Complete psychosocial assessments and provide emotional support to residents and families.

- Arrange home care, medical equipment, transportation, and community services.

- Communicate with hospitals, physicians, insurance companies, and outside providers.

- Maintain accurate documentation and ensure compliance with state and federal regulations.

- Participate in care plan meetings and collaborate with the interdisciplinary team.

Qualifications

- MSW preferred; LMSW/LCSW preferred.

- Experience in skilled nursing, rehabilitation, hospital, or case management preferred.

- Strong communication, organizational, and teamwork skills.

- Knowledge of Medicare, Medicaid, and discharge planning is a plus.
