MDS Director

Atlantic Coast Rehabilitation and Healthcare Center, a 160-bed skilled nursing facility in Ocean County, New Jersey, is seeking an experienced MDS Director (RN) to lead our MDS program. If you have a strong background in MDS, care planning, and regulatory compliance, and enjoy collaborating with an interdisciplinary team, we'd love to hear from you. 

Key Responsibilities: 

• Manage the scheduling, completion, and accuracy of all MDS assessments using Point Click Care (PCC). 

• Ensure compliance with all State and Federal MDS regulations. 

• Submit MDS assessments electronically in a timely and accurate manner. 

• Lead and participate in weekly utilization review meetings with the interdisciplinary team. 

• Develop, review, and update individualized resident care plans. 

• Educate and support staff on MDS processes and documentation requirements. 

• Monitor Quality Measures (QM) and implement corrective actions to improve outcomes. 

• Maintain accurate ICD-10 coding for residents. 

• Participate in facility meetings and assist with other clinical responsibilities as needed. 
   

Compensation: 

• $90000-$105000 per year and is based on your experience 

Qualifications:

• Current New Jersey Registered Nurse (RN) license in good standing (Required). 

• 2 years of recent MDS experience in a skilled nursing facility preferred. 

• Experience using PointClickCare (PCC) requried 

• Strong understanding of Medicare, Medicaid, MDS assessments, RAI process, and care planning. 

• Excellent communication, organizational, and leadership skills.
