About the Role:
Som Medical Practice is looking for a passionate and skilled Nurse Practitioner to join our growing team in Niagara Falls, Olean, Lockport, and Hamburg.  This is an exciting opportunity to make a meaningful impact on patient lives in a supportive and collaborative clinical environment. Responsibilities:

• Conduct comprehensive patient assessments, including physical exams, medical histories, and diagnostic evaluations
• Diagnose and treat acute and chronic illnesses in alignment with evidence-based clinical guidelines
• Develop and manage individualized patient care plans and treatment protocols
• Order, interpret, and follow up on laboratory tests, imaging, and other diagnostic studies
• Prescribe medications and provide patient education on medication management
• Collaborate with physicians, specialists, and care team members to ensure continuity of care
• Maintain accurate and timely documentation in the electronic health record (EHR) system

Requirements:

• Active Nurse Practitioner (NP) license in the state of New York
• Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program
• Current board certification (FNP-C, ANP-C, or relevant specialty certification preferred)
• Valid DEA license and prescriptive authority in New York State
• Experience in a primary care, internal medicine, or outpatient clinical setting preferred
• Proficiency with electronic health record (EHR) systems
• Strong interpersonal and communication skills with a patient-centered approach
