About the Role:

As a Social Service Designee, your primary responsibilities will include:

• Conducting social service assessments in Point Click Care and maintaining accurate records

• Coordinating ancillary services for all residents, including discharge planning and equipment ordering

• Identifying and addressing social service concerns that may arise for each resident

• Collaborating with the administrator to ensure effective communication and resolution of concerns

• Maintaining organization and prioritizing tasks to meet deadlines

Requirements:

• High school diploma or equivalent required

• Ability to work independently and as part of a team

• Excellent communication and documentation skills

• Strong organizational and problem-solving skills

• Ability to maintain confidentiality and handle sensitive information
