Essential Duties and Responsibilities

  • Conduct street, shelter, hospital, and community-based outreach to identify and engage vulnerable individuals in need of services.
  • Perform comprehensive psychosocial assessments and develop individualized service plans.
  • Build rapport and establish trusting relationships with clients utilizing a trauma-informed and person-centered approach.
  • Provide crisis intervention, counseling, and supportive services to individuals experiencing behavioral health and social service needs.
  • Coordinate referrals and linkages to housing, healthcare, mental health treatment, substance use treatment, benefits, employment, and other community resources.
  • Collaborate with multidisciplinary teams, community providers, hospitals, shelters, and government agencies to ensure continuity of care.
  • Assist clients in obtaining identification, benefits, insurance coverage, and other essential documentation.
  • Monitor client progress and adjust service plans as needed.
  • Maintain accurate and timely documentation in electronic health records and case management systems.
  • Participate in case conferences, team meetings, and community outreach initiatives.
  • Track outreach activities and program outcomes in accordance with agency and funding requirements.
  • Provide advocacy on behalf of clients to address barriers to housing, healthcare, and social services.
  • Ensure compliance with agency policies, HIPAA regulations, and applicable federal, state, and local guidelines.

Qualifications


Licensure

  • Licensed Master Social Worker (LMSW) in New York State required.

Experience

  • Minimum of 2 years of experience working with individuals experiencing homelessness, serious mental illness, substance use disorders, or other vulnerable populations preferred.
  • Experience providing outreach, case management, housing services, or behavioral health services strongly preferred.
  • Knowledge of community resources, housing programs, and public benefits systems preferred.
