What You'll Do

• Coordinate medical, behavioral, and social care for clients across NYC boroughs
• Conduct in-person and remote assessments and health screenings
• Develop, implement, and monitor individualized care plans
• Connect clients with housing, behavioral health, substance use, and community support services
• Advocate for clients across healthcare systems and community programs

Qualifications

• Current NYS RN license
• Bachelor's Degree in Nursing or equivalent experience
• 1–2 years of clinical experience, including care management experience, preferably in behavioral health or community care
• Strong assessment, documentation, and communication skills
• Spanish or another second language is a plus
