Physcial Therapist And Physical Therapist Assistant

The Physical Therapist will provide skilled home‐based physical therapy services, perform comprehensive assessments, develop individualized care plans, and collaborate with the care team to promote optimal patient outcomes.

Key Responsibilities

• Conduct initial and ongoing patient assessments
• Develop and implement individualized treatment plans
• Provide therapeutic exercises, gait training, balance training, and functional mobility interventions
• Educate patients and caregivers on safe mobility and home exercise programs
• Document all care in accordance with state, federal, and agency standards
• Communicate effectively with physicians, nurses, and interdisciplinary team members

Qualifications

• Active Florida Physical Therapist (PT) license
• Current CPR certification
• Home health experience preferred but not required
• Strong clinical judgment, communication skills, and professionalism
• Reliable transportation for home visits
