Job Description
Position Title: Home Health Aides
Location: Nassau & Suffolk Counties (Multiple Location)
Duration: 2 years
Work Schedule: Multiples shift flexible with schedule
Responsibilities for this position include, but are not limited to:
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility.
- Support individuals in developing independent living skills and achieving person-centered goals.
- Monitor and report changes in health, behavior, or emotional status to nursing staff or supervisors.
- Document daily care, activities, and observations accurately.
- Assist with meal preparation, feeding, and adherence to prescribed diets.
- Perform light housekeeping, laundry, and maintain a clean and safe living environment.
- Accompany individuals to medical appointments, community activities, and recreational outings.
- Follow Individualized Service Plans (ISP) and behavior support plans.
- Respond appropriately to emergencies and follow established protocols.
- Work collaboratively with nurses, therapists, supervisors, and interdisciplinary team members.
To be considered for this position, you should have: [Skills, Education, or Experience]
- Minimum 1 year of Home Health Aide, Direct Support Professional (DSP), CNA, or residential care experience.
- Experience working with individuals with developmental disabilities, autism, or behavioral health is preferred.
- Strong communication, teamwork, and documentation skills.
- New York State Home Health Aide (HHA) Certificate (required)
- Valid CPR/BLS Certification (American Heart Association or client preferred)
- Valid First Aid Certification (preferred or required by facility)
- Valid New York State Driver's License (preferred or required for community outings/transportation)
