Case Manager RN - Utilization Review + Discharge Planning

Care Coordination

• Perform medical + psychosocial assessments
• Create and manage discharge plans
• Coordinate with MDs, nurses, and social workers
• Lead care planning discussions

Utilization Management

• Review medical necessity & admission status
• Ensure correct level of care (IP vs Observation)
• Communicate with insurance/payors
• Escalate authorization/status issues

Throughput & Discharge

• Identify discharge barriers early
• Improve patient flow & length of stay (LOS)
• Arrange post-discharge services (rehab, home care, etc.)

Compliance & Documentation

• Follow regulatory + hospital guidelines
• Maintain accurate documentation
• Report safety or quality concerns

Requirements

• Option 1: 3+ years recent Case Management (acute care)
• OR Option 2: 3+ years RN in:
• • Med/Surg
  • Telemetry
  • ICU / PCU
  • ED
  • Neuro

Conditional Candidates

• Home Health / Insurance CM → ONLY if:
• • 3 years total acute care experience
  • • 1 year acute care within last 5 years
• Active FL RN license or compact

Compensation Breakdown

• Base Pay Range: $31.94 – $43.92/hr

Shift Differentials (Stackable)

• Evening: +$2.50/hr
• Weekend: +$2.00/hr
• Max Add-On: +$4.50/hr

Example: Evening + Weekend = base + $4.50/hr

Sign-On Bonus ($10K Total)
