About the Opportunity

We are seeking a Registered Nurse Coordinator to join a specialized healthcare team dedicated to serving individuals and families affected by bleeding disorders.

In this role, you'll have the opportunity to build meaningful relationships with patients while providing comprehensive nursing care, patient education, treatment coordination, and advocacy. You'll work alongside a multidisciplinary team committed to delivering exceptional care in a supportive, patient-focused environment.


Responsibilities

  • Conduct nursing assessments and ongoing patient evaluations
  • Provide telephone triage and clinical support to patients and families
  • Develop, implement, and maintain individualized treatment plans
  • Coordinate care between patients, providers, hospitals, pharmacies, and community resources
  • Administer medication infusions and other clinical treatments
  • Provide patient and family education regarding bleeding disorders and treatment options
  • Participate in comprehensive clinic visits and collaborate with providers on patient care plans
  • Process medication refill requests as appropriate
  • Maintain accurate and timely patient documentation in EPIC
  • Serve as a patient advocate and resource for care coordination needs
  • Participate in an on-call rotation (approximately 4-5 days every other month)


Required Qualifications

  • Active Registered Nurse (RN) license in Wisconsin or eligible through reciprocity
  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
  • Current CPR certification for Healthcare Providers
  • Minimum 3 years of nursing experience
  • Experience using EPIC electronic medical records
  • Strong communication, organization, and patient education skills
  • Ability to work independently while collaborating within a multidisciplinary team


Preferred Qualifications

  • BSN preferred
  • Outpatient clinic experience
  • Hospital nursing experience
  • IV infusion experience
  • Experience with specialty care or chronic disease management
