RN Case Manager - Acute Care Hospital (Day Shift)

Role: Case Manager RN
Location: Tallahassee, FL
Facility Type: Acute Care (288-bed hospital)
Reports to: Director of Case Management
Schedule:

• Day Shift: 8:00 AM – 4:30 PM
• Weekend: 1 in every 4 weeks

Care Coordination

• Perform psychosocial + medical assessments
• Build and manage individualized discharge plans
• Coordinate across interdisciplinary teams (MDs, nurses, social work, etc.)
• Lead care planning meetings

Utilization Management

• Review medical necessity + admission status
• Ensure correct level of care (inpatient vs observation)
• Communicate with insurance / payors
• Escalate status issues when needed

Throughput & Discharge

• Identify barriers to discharge
• Drive patient flow / LOS reduction
• Arrange post-discharge services

Compliance & Quality

• Follow regulatory + HCA compliance standards
• Document all interventions clearly
• Report patient safety issues

Requirements

Experience

3+ years RECENT Case Management in acute care
OR
3+ years RN experience in:

• Med/Surg
• Telemetry
• ICU / PCU
• ED
• Neuro

Recent = within last 12 months

Alternative Path (conditional):

• Home Health / Insurance CM acceptable ONLY IF:
• • 3 years total acute care experience
  • At least 1-year acute care in last 5 years

Shift Differentials

• Evening Shift: +$2.50/hr
• Weekend Shift: +$2.00/hr

If working evening + weekend, candidate earns base + $4.50/hr extra.
