Case Manager RN - 3x12s Day Shift

Summary:
The Case Manager RN coordinates patient care across the continuum, focusing on medical necessity, appropriate level of care, resource utilization, and safe discharge planning. This role works closely with physicians, interdisciplinary teams, patients, and families to ensure efficient, high-quality outcomes while managing length of stay and barriers to care.

Responsibilities:
• Perform comprehensive psychosocial and medical assessments
• Develop and document case management care plans
• Coordinate interdisciplinary team meetings and care planning
• Review admissions for medical necessity and level of care appropriateness
• Evaluate observation vs inpatient status
• Conduct utilization and payer reviews
• Facilitate discharge planning and post-acute services
• Identify barriers to care and improve patient throughput
• Educate patients and families on post-discharge resources
• Ensure compliance with regulatory and hospital standards
• Support high-risk and under-resourced patient populations

Requirements:
• Active FL RN license (or compact with FL within 90 days)
• ADN or Diploma in Nursing required (BSN preferred)
• 3+ years recent acute care experience OR 3+ years Case Management experience
• Strong experience in Med-Surg, Tele, ICU, PCU, Neuro, or ED preferred
• Case Management or Utilization Review experience preferred

Compensation:
• $31/hr – $43/hr based on experience
• Evening Differential: +$2.50/hr
• Weekend Differential: +$2.00/hr (stackable, up to +$4.50/hr)
• Sign-On Bonus: $10,000 (paid over 24 months)
